This is a recent case I worked on in mid June. I used Cerec technology to create a crown and deliver it in the same visit. The patient did not need a temporary, an impression, or a second visit. A normal crown usually takes up to three weeks to get back from the lab. A normal crown may even require the tooth to be numbed at the second visit to deliver the crown. Cerec crowns shouldn't be used for every crown, but they can be handy. Here is a tooth I treated a couple of weeks ago. The tooth has a visible crack and a large filling.

I prepared the tooth for the crown.

We spray a reflective powder on the tooth and take a picture of the tooth. The pictures of the preparation then render a 3-D model on which we design the crown.

The crown is made while the patient waits (aprox. 15 minutes). Then we deliver the crown and the patient is ready to go.

The patient has undergone all of the treatment in one appointment. No impression. No temporary. No need to be numbed again.

The sushi and crab cakes look great. I think the store will be a great asset to Fredericksburg. It was nice to see so many people shopping in a down economy, and so many people with new jobs!

Over the past couple of years more and more dentists have moved away from amalgam "silver" fillings and instead place composite "tooth colored" fillings. These types of fillings and filling materials have improved greatly over the years and can be used to fill a wide variety of cavities. Composite restorations are usually resin based and require adhesives to be placed. They come in many shades and can usually be matched to the existing tooth very well. Many people ask me if these types of fillings are better than amalgam fillings, but that is a loaded question. I think both have their purpose, and if used correctly, can make a great restoration. Here is a composite restoration I placed just the other day.
The decay is shown in the red circle. Decay is softer and less dense than tooth so it shows up as a dark shadow on the x-ray.

The decay can be seen on the top of the tooth. Decay often starts in natural fissures on the tooth surface.

The decay is removed with a high speed dental hand piece. We now place a dental etch, which will clean the surface and prepare it for dental adhesives. After the etch we place a primer, which will make the tooth more receptive to adhesives. Finally, we will place the bonding adhesive and cure it with a special ultraviolet dental light.

The final restoration is placed, cured and polished. The color matches well and looks very natural. The restoration is ready to chew!!

Tooth sensitivity

Summertime is quickly approaching and many people will be eating ice cream and frozen treats. These are usually a big offender of sensitive teeth. There are some simple ways to help calm the sensitivity. Sensodyne and other products are great, but many people don't use them correctly. These products must stay on the teeth for an extended time. When using the Sensodyne brush it on and let it sit. The longer it stays on your teeth the better the result. When sensitivity persists come see us. We distribute a prescription toothpaste with at 1.1 % fluoride. This is almost ten times as much as toothpaste. We can also make trays which are used to hold fluoride and can be worn at night. In these troubled financial times everyone is looking to save a penny or two. Many times people think they can avoid the dentist to save a little money, but that can wind up costing you in the end. Now is the time to take care of your teeth to avoid costly trips to the dentist later on. Here are a couple of tips to save you money at the dentist:
1) find a dentist you trust and is looking out for your best interest. A dentist you can trust will think about you as a person with needs. They will be sensitive to your situation, and will be willing to work with you.
2) Avoid dental "chains". These are dental offices that are run by a corporation and often have financial goals to achieve every day.
3)Avoid hidden charges. Some dental "chains" may offer free bleaching but then add fluoride treatments or other elective procedures to boost their production on to each patient.
4)Avoid sodas and sugary drinks. Fast food restaurants always do well when the economy is bad because the food is cheap. Make sure you drink water or diet drinks if you are going through the drive through.
5)Ask if you can skip a six month exam, but still get the cleaning. It is important to get your six month cleaning to maintain your dental health. I do not recommend doing this all the time, but if you are going to cancel your appointment all together for financial concerns, take the less expensive route and get your teeth cleaned. The dentist is required by law to do at least one exam a year, so you won't be able to do this all the time.
6)Increase your home care. You play a major role in keeping your teeth healthy. Regular and thorough home care can help you avoid costly dental treatment.
7)Get a second opinion. Many dental "chains" will give you a speedy and sometimes costly treatment plan. Check with a local established dentist to get a second opinion.
8)Make use of your insurance. You don't always have to go to a dentist that accepts your insurance. Many insurances will pay for cleanings and exams if you go to dentists who will file your insurance.
9)Check google, yelp and other web sites to get reviews on dentists. A dentist with many good reviews must be doing something right.
10)Go with your gut. If you feel pressured or don't feel right about a dentist don't continue to got there. You will probably wind up spending more money and not feel secure. You will know when the treatment feels right.

Giacomo is feeling better

Many of our patients in Fredericksburg are familiar with our mascot Giacomo. Giacomo took ill on Tuesday and gave us a pretty good fright. After several trips to the vet he is feeling some better, but is not himself just yet. We always appreciate treats and gifts for Giacomo, but the good life does not agree with him. Giacomo will have to have to keep a bland diet from now on. Giacomo will be up for greeting everyone soon. We hope everyone has a fantastic holiday.
